What a clinic form should actually ask
Fields chosen to schedule a visit while steering medical detail to the consultation:
| Field | Why |
|---|---|
| Name and phone | Clinics confirm by phone more than email; take both, require one. |
| For the confirmation and the calendar invite. | |
| Appointment type (dropdown) | First visit, follow-up, specific service: it routes to the right calendar. |
| Preferred days or times | A text field beats a fake calendar; the receptionist matches it to the real one. |
| Note, explicitly limited | Label it "anything we should know (no medical details, please)": it sets expectations and keeps diagnoses out of the inbox. |
Every field you add costs completions, so this list is deliberately short; everything else is a question for the reply. The snippet below is the whole form; build a styled variant in the free form generator or start from a template.

</form>Three places the form can live
On the practice website, or as a hosted page linked from the Google profile where many patients actually find you:
- On your website: paste the form where the enquiry should happen. It is your HTML, so it inherits your design; the HTML docs list the hidden fields (redirect, subject, language).
- Without a website: every form also exists as a hosted page at
formgong.com/f/<key>, built from the same saved design, plain HTML. That link goes in an Instagram bio, a Google Business Profile booking link, or a message, and submissions arrive exactly like the embedded form's. - From a preset: the clinic appointment template creates the form with these fields in one click, connected to your account.
Where the enquiry lands, and how fast you see it
Each submission is delivered to email and, on every plan including Free, to Telegram, with Reply and Mark handled buttons in the chat. Reception sees requests in email; the practice owner can additionally get Telegram pings for same-day gaps worth filling. Every submission also sits in the dashboard inbox with CSV export, so a filtered email is never the only copy, and the free plan takes 1,000 submissions a month, which covers most small businesses with room to spare.
Spam is filtered server-side before it reaches you: a honeypot field, content checks, and optional Cloudflare Turnstile you switch on per form without touching the code. Flagged submissions land in a Spam folder you can review, so a real enquiry misjudged by the filter is one click away, not lost.
Health data, said plainly: what we do and do not claim
The honest part. Formgong stores submissions in Cloudflare D1 under EU jurisdiction, offers a DPA, shows a privacy notice with your policy link under every form, and never forces a consent checkbox where law does not require one; retention is configurable per form. What Formgong does not claim: HIPAA compliance. A US practice that needs a Business Associate Agreement for protected health information should use a healthcare-specific vendor for anything medical and keep a general form for scheduling-only requests, which is exactly what the field design above produces. For EU practices under GDPR, an appointment request with the "no medical details" framing is the pattern our own GDPR form-types guide describes: minimal data, a clear purpose, a linked policy.

Questions
What should a clinic contact form ask?
Name, phone (required) and email, an appointment-type dropdown, preferred days or times as free text, and a note field explicitly labelled to exclude medical details. The goal is scheduling; the medical conversation belongs in the consultation, not in email.
Is Formgong HIPAA compliant for medical forms?
No, and we say so plainly: Formgong does not claim HIPAA compliance or sign BAAs. US practices handling protected health information need a healthcare-specific vendor for that; a general form fits scheduling-only requests designed to avoid medical detail.
How does a clinic form handle GDPR?
By data minimisation and transparency: ask only what scheduling needs, label the note field to exclude medical details, and show the privacy notice with your policy link, which Formgong renders under every form. Submissions are stored in the EU, with a DPA available, and per-form retention settings.
Can patients book appointments directly through the form?
The form takes requests with preferred times; the confirmation happens by phone or email against the real calendar. Clinics that want true slot booking need a scheduling system; the form then covers everything that is not a bookable slot.
