A contact form for lawyers and law firms

Legal intake forms have an occupational hazard: prospective clients volunteering case details to a stranger's inbox before any engagement exists. A good law-firm form asks for the matter type and urgency, takes contact details, and explicitly tells the visitor not to include confidential detail, protecting both sides before the first call.

Short answer: A law firm contact form should ask for name, contact details, the matter type (dropdown), urgency, and a brief note, with a visible line telling visitors not to include confidential case details, since a form submission creates no attorney-client relationship. Formgong renders your privacy notice under the form, stores submissions in the EU with a DPA, and delivers to email and Telegram for the conflicts check before anyone calls back.

What a law firm form should actually ask

Fields for triage, with the guardrail that matters in this profession:

The fields, and why each one earns its place
FieldWhy
Name, email, phoneThe callback happens by phone; documents move by email.
Matter type (dropdown)Family, employment, property, business, other: routes to the right lawyer and surfaces conflicts early.
Urgency"Court date next week" and "general advice" need different queues.
Brief note, with the warningLabel: "briefly, what is it about — please do not include confidential details". The intake interview happens after the conflicts check, not in the form.

Every field you add costs completions, so this list is deliberately short; everything else is a question for the reply. The snippet below is the whole form; build a styled variant in the free form generator or start from a template.

</form>

Three places the form can live

On the firm's site, or as a hosted page for directories and profiles that allow one link:

  • On your website: paste the form where the enquiry should happen. It is your HTML, so it inherits your design; the HTML docs list the hidden fields (redirect, subject, language).
  • Without a website: every form also exists as a hosted page at formgong.com/f/<key>, built from the same saved design, plain HTML. That link goes in an Instagram bio, a Google Business Profile booking link, or a message, and submissions arrive exactly like the embedded form's.
  • From a preset: the legal inquiry template creates the form with these fields in one click, connected to your account.

Where the enquiry lands, and how fast you see it

Each submission is delivered to email and, on every plan including Free, to Telegram, with Reply and Mark handled buttons in the chat. Intake speed wins engagements here too, with a twist: the partner can see the matter type in Telegram and run the conflicts check before anyone commits to a callback. Every submission also sits in the dashboard inbox with CSV export, so a filtered email is never the only copy, and the free plan takes 1,000 submissions a month, which covers most small businesses with room to spare.

Spam is filtered server-side before it reaches you: a honeypot field, content checks, and optional Cloudflare Turnstile you switch on per form without touching the code. Flagged submissions land in a Spam folder you can review, so a real enquiry misjudged by the filter is one click away, not lost.

Privilege, confidentiality and what email honestly is

Said plainly: a website form submission is not privileged communication, no attorney-client relationship exists at that point, and ordinary email is not an encrypted channel, which is exactly why the note field carries its warning and why firms should repeat it in a line under the form (Formgong shows your privacy notice and policy link there on every form). Formgong stores submissions in Cloudflare D1 under EU jurisdiction, offers a DPA, and lets you set per-form retention so intake notes do not live forever by accident. For the engagement itself, your practice-management system and its secure channels take over; the form's job ends at a clean, conflict-checkable intake request.

Questions

What should a law firm contact form ask?

Name, email, phone, a matter-type dropdown, an urgency level, and a brief note whose label explicitly tells the visitor not to include confidential details. Enough to run the conflicts check and route the callback, nothing that creates risk in an inbox.

Is a contact form submission protected by attorney-client privilege?

No. No engagement exists when a prospective client submits a website form, and the submission travels like ordinary email. That is why the form should visibly warn against including confidential case details and why firms state it under the form.

How should law firms word the form disclaimer?

Short and visible: that submitting the form does not create an attorney-client relationship, and that confidential details should wait for the engagement. Formgong renders your privacy notice with your policy link under every form; the disclaimer line fits the same spot.

Where are legal intake submissions stored?

In Formgong's case: Cloudflare D1 under EU jurisdiction, with a DPA available, per-form retention you control, delivery to email and Telegram, and CSV export. The engagement's confidential material belongs in your practice-management system, not the intake form.
